- Form D - Change of Regular Transportation Form For security and safety reasons, any student who is taking alternative transport should complete and submit this form one day in advance to the Summer Program Office.
- Form E - Notification of Parents absence from Hong Kong To be completed if parents/guardians are away at any time during the enrolled session.
- Student ID TAG (one tag per student) If your child/children are younger than 9 years old, please download and fill in, cut out and put a string through the punch hole and wear the ID tag around his/her neck or put the tag in a clear badge and pin it on their shirt. This is important so that young children can be appropriately escorted with minimal confusion.
- Walker Registration (Grade 6 and above only) - Any students from Grade 6 and above must register as walkers if they want to be a walker during the Summer Discovery Program. Only those MS and HS students registered on our dismissal list as a walker will be allowed to depart through this exit.
