The Change of Regular 4pm Request form allows parents to request to change their child's regular 4pm dismissal. Please request any change at least 24 hours before your child's ASA. If you have to make an urgent change on the day of your child's ASA please submit the form by noon on the day and send a follow-up email to the Activities office.
All approved requests will be confirmed in a follow-up email from the Activities office. These changes will automatically be reflected on the daily dismissal sheets provided to Activity leaders.
